Stained Glass Class Waiver and Safety Requirements
Please carefully read the safety requirements and waiver below. Participant safety during classes and events are my top priority and our activities for minors are thoughtfully designed with safety in mind. The vast majority of our classes for young crafters include safely handling glass of various sizes (some are as fine as sand) under instructor/adult supervision. They are taught to use gentle hands, and all participants will be provided with safety glasses and any other appropriate Personal Protective Equipment. Older minors may participate in adult classes on a case by case basis. (please reach out to request instructor approval). Safety Requirements
Class participants under the age of 18 are required to have an adult present with them when participating in a class designed for adults.
It is recommended that class participants are dressed appropriate for their scheduled class. (long hair tied back if soldering, closed toe shoes, comfortable clothing that does not dangle or restrict movement.)
Participants should not use any equipment or materials without an instructor present, and use all provided safety gear if recommended. (safety glasses or gloves)
Traditional stained glass solder contains lead. Lead-free solder is available if desired, however this may incur an additional material fee). Pregnant individuals, should consult their doctor with any concerns they may have. Older students may participate in soldering in small amounts with adult permission and supervision. Note that lead in solder doesn’t easily pass through intact skin in significant amounts, but the primary risk is ingestion or inhalation of lead dust/fumes. Participants are always encouraged to wear gloves and wash their hands thoroughly after soldering to prevent lead particles. Wearing nitrile gloves while soldering is a best practice in our studio, so lead exposure is quite limited when following instructor directions.
Never consume food or drink while soldering or when applying chemical patinas. The greatest risk of lead exposure is through ingestion. With proper precautions however, working with stained glass solder can be quite low risk. Participants will use gloves and thoroughly wash hands after soldering and after coming in contact with chemical patinas. The soldering and patina station will be a NO FOOD zone.
Stained glass class participants are required to complete and sign a liability waiver, and read and agree to abide by the safety procedures as outlined by Say It With Glass Designs, L.L.C. Participants under the age of 18 must have approval from the instructor as well as a parent or guardian to enroll in stained glass classes and complete the required minor waiver.
Release and Waiver of Liability Agreement (Minor Child)
I hereby certify that I am the adult parent or guardian of the child named below, a minor child under the age of eighteen years, and I consent to his/her participation in Activity at the property and is willing to permit me to sign this Agreement on their behalf to participate in the Activity, upon the terms and conditions of this Agreement. All or some of the Activity may take place on the Company’s premises or property (“Premises”) located at 101 Two Hills Drive, Unit 180, Carrboro, North Carolina 27510. I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of serious bodily injury, property loss or damage) of said minor’s participation in activities at the Premises. I recognize my responsibility to ensure that said minor child participates only in those activities for which he/she has the required skills, age, and/or qualifications. I understand that Company shall have no responsibility to pay for medical treatment and related costs if said minor child is injured. I further understand and agree that the Company supplies the materials “as is”, and that the Company disclaims all warranties, express or implied, including warranties of merchantability and fitness for a particular purpose. Knowing the risks described above, I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s use of the Premises and materials, resulting from any cause whatsoever, and regardless of fault. I am at least eighteen years of age and have carefully read and freely signed this Release and Waiver of Liability Agreement (Minor Child). I understand and agree that no oral or written representations can or will alter the contents of this document. I agree that this agreement shall comply with all applicable federal, state, regional and local laws, rules and regulations of North Carolina.
I HEREBY ACKNOWLEDGE THAT I HAVE FULLY READ AND UNDERSTAND EACH OF THE ABOVE PROVISIONS AND THAT I UNDERSTAND I AM GIVING UP SIGNIFICANT LEGAL RIGHTS ON MINE, INCLUDING THE RIGHT TO SUE THE COMPANY. I ACKNOWLEDGE THAT PRIOR TO SIGNING THIS AGREEMENT I HAD THE OPPORTUNITY TO CONSULT WITH AN ATTORNEY TO REVIEW THIS AGREEMENT. I AM AT LEAST EIGHTEEN (18) YEARS OF AGE AND FULLY COMPETENT, AND I EXECUTE THIS AGREEMENT VOLUNTARILY AND FOR ADEQUATE CONSIDERATION INTENDING TO BE FULLY BOUND.
